Information For New Patients
On your first visit to see us, we'll ask you to complete a health questionnaire and travel itinerary form. Patients usually spend anywhere between 10 and 20 minutes completing this document. To speed up your visit, we have made this form available for download from our website. That way, your time at our office can be better spent discussing your travel needs.
